Eyelid lift, clinically known as blepharoplasty, is a cosmetic surgery procedure to address the signs of aging around the eyes and restore a rejuvenated eyelid appearance. Eyelid lift surgery can be performed on either lower or upper eyelids, or both. The procedure is sometimes performed in conjunction with brow lift or facelift to create more harmonious outcomes.

Candidacy
The surgeon will carefully assess the patient’s eyelid anatomy to determine the presence of loose skin, fat tissue pockets, asymmetry, and under eye bags. Depending on this evaluation and the patient’s personal cosmetic goals, the surgeon will recommend upper or lower eyelid cosmetic surgery.
Most candidates for blepharoplasty are in their 40s or 50s, a younger patient with droopy eyelids due to genetic reasons may also receive this procedure. A good candidate should be healthy and not suffering from any major eye disease.
Upper Eyelid Lift Procedure
The upper eyelid surgery procedure will begin with the surgeon marking the natural lines and creases of the upper lids where the incisions will be placed. This will keep the scarring very discreet. The surgeon will carefully map out the loose skin before the surgery. Once the patient is placed under anesthesia, the surgeon will create an incision in the marked site.
Loose skin and excess fat will be removed through the incision, and thereafter the incision will be carefully closed with fine sutures. Incisions placed in natural folds often heal well and scarring is barely visible after healing.
Lower Eyelid Lift Procedure
Depending on the patient’s unique lower lid anatomy, the surgeon will choose the appropriate lower blepharoplasty technique. If fat pockets need to be removed, but the skin is fairly elastic, the surgeon may adopt a trans-conjunctival approach to eliminate the fat. This is a less invasive surgery with an incision created on the back side of the eyelid, leaving no external scar.
If only excess skin needs to be removed from the lower lid, the surgeon will create an incision just below the eyelash area. The scar typically heals well. If both fat and loose skin needs to be removed, the two techniques may be combined.
Canthopexy
In some cases, the lower eyelid support may become weaker as the eye ages. This can cause the lower lid to become stretchy or floppy. Surgery on the lower eyelid in such a case has the risk to create a pulled down eyelid. Therefore, the surgeon may perform canthopexy, which is an internal suture suspension, to reduce this risk.
