What are Dermal Fillers?

What are the different kinds of dermal fillers?

Recently, we have seen the introduction of several new FDA-approved dermal fillers.  And, of course, we have them all available for you!  Each one has specific uses where they really do well.

  • Juvéderm® and Restylane®: These are the original hyaluronic (HA) gel products that last for up to 6–12 months.  They are often used for correcting the nasolabial folds, and for lip plumping.
  • Refyne® & Defyne®:  Latest generation HA gels, which are more flexible, resulting in a more natural appearance with movement.  Defyne is specifically designed for deep wrinkles.
  • Restylane Lyft®: Formally known as Perlane, this gel is designed for deeper injection than Restylane.  It works well for severe facial wrinkles and folds.
  • Voluma®: An HA gel specifically formulated for filling and lifting of the cheek areas.
  • Vollure®:  A new generation HA gel specifically formulated for deep wrinkles.
  • Restylane Silk: An HA gel specifically formulated for the lips and for fine lines.  It can also be used, off-label, in the tear trough area.  It’s a favorite of ours for natural-appearing lip enhancement.
  • Sculptra®: More a volume-stimulating agent than a filler, Sculptra is made of poly-L-lactic acid, a liquid form of surgical suture material. It gradually stimulates your body to make new collagen in the treated areas. It is excellent for re-volumizing sunken areas of the face – especially the cheeks, the eyebrow and temple areas.
  • Kybella:  This is a fat-melting agent, not a dermal filler.  It is used to treat chin (submental area) fullness, as an alternative to liposuction or CoolSculpting.
  • Rejuva:  This is a new filler, known technically as adipose-derived extracellular matrix.  It acts like an “off-the shelf” fat graft.

Fiala Aesthetics participates in Allergan’s “Brilliant Distinctions” Program, a customer loyalty program in which you earn points for discounts on future purchases of Botox, Latisse, Juvederm and other Allergan products.

We also participate in Galderma’s “ASPIRE” Program, for purchases of the Restylane family of products.  Think of them like  “frequent flyer” programs!

Are dermal fillers safe?

Yes. Allergic reactions to dermal fillers are rare. The track record of safety with the injectable HA gel products is excellent, with a very low rate of complications.

How do I decide which filler to get?

Each filler has specific features that are suited to certain facial applications. Some need to be placed deeply in the tissues, others can work right underneath the skin surface. Some can be placed in the lips or near the eyes – and some can’t. Come and see us for an in-person consultation, and we’ll help you sort this out.

Will I bruise?

You might. There are a lot of little vessels around the eyes and mouth – and it’s hard to totally avoid all of them.  You can help to minimize bruising by avoiding all the things that increase bruising – like aspirin, motrin, advil, Vitamin E, fish oil supplements and alcohol.  Also – don’t have fillers within 3 weeks prior to any important social events.  Injections near the eyelids and lips do tend to bruise more often, even with a careful injection technique.

Are dermal fillers painful?

Usually, people tolerate them very well. Most of the modern fillers have lidocaine mixed in the gel, which numbs the area upon injection. Also, numbing cream and ice can be used, to help make the injection be more comfortable. Let us know if you are needle-phobic or highly anxious – there are some additional things we can do, with advance notification.

Who does the actual injecting?

You have a choice of two excellent injectors!  Both Phi, our Certified Physician Assistant, and Dr. Rhee perform injections.  They both have specific training and experience in this area.  Phi, for example, has been recognized by Galderma as one of Florida’s top injectors.

Will dermal fillers replace a facelift?

No. Dermal fillers do not lift or tighten tissues, like a facelift would. But fillers work very well indeed for the patient who either notices a few lines & wrinkles and is not ready for a facelift, or for the patient that has already had a facelift and wants to maintain the improved appearance for as long as possible. Botox, dermal fillers, skincare and surgery each play a role in helping treat appearance issues.

Will I have a lot of swelling?

It depends on the product used, the amount of product used, and your body. There is often some swelling for the first 2 or 3 days, after which things usually settle down, giving the desired appearance.  True allergies to these products are actually very rare, but please inform us if you have ever been told that you have an allergy or sensitivity to an injectable filler product.

What if I change my mind after having the dermal fillers?

One of the nice things about the HA group of fillers, like Juvederm, and Restylane, is that we can “undo” them, if necessary. There is an injectable agent that can “erase” the fillers if you don’t like them, or there are any issues with them.  Permanent fillers, like Artefill, are not reversible.

Can one syringe of product treat my whole face?

Honestly, no.  This would be like asking one quart of paint to cover your entire house.  It would be better to select one area and correct that zone properly, rather than spreading the filler out “a little bit here, and a little bit there”.

How much will it cost?

It depends on the products used, and the amount of each product to get a good correction, so it can be quite variable from person to person. It may take more than one treatment session to achieve complete correction, as well.  If this is your first treatment with us, please ask us for an estimate, so you’ll know what to expect financially.

What is Kybella?

Kybella is an FDA-approved injection for melting fat.  Currently, it is offered only for the “double-chin” area, but other uses are being investigated.  A series of injections are performed, depending on the configuration of the individual’s fatty deposits in the neck.  Significant improvements in neck fullness, but not skin laxity, can be seen with this treatment.
